Participating Dental Insurance Companies
As a courtesy to our patients Queensgate Dental participates in many dental insurance plans (PPO). We will submit claims on your behalf to and have the reimbursement sent to us. Any conversation with the insurance company is the patient’s responsibility as is any unpaid balance. All service fees are ultimately the patient’s responsibility.
For those plans we are not a participating provider (out of network) we require payment in full by the patient. We will submit all claims on your behalf and the insurance company reimburse the patient. Any conversation with the insurance company is the patient’s responsibility as is the fee for services provided.
For any procedure that may be covered by your medical insurance we suggest you contact your insurance company prior to treatment. Following treatment we will provide you with a treatment invoice to submit for reimbursement directly to you.
Please contact your insurance company to confirm that we are a provider. Please bring all insurance information with you to your appointment. Deductibles, Co-payments and non-covered service fees are expected at time of treatment. All service fees are ultimately the patient’s responsibility.
